Heads Up Somerset is a mental health & wellbeing charity supporting clients with their mental wellbeing and those suffering with dementia. We are a small, friendly team who put the people we support at the heart of everything we do.

We are delighted that due to growth within our charity, we are recruiting for experienced Community Outreach Support Workers who can provide an energetic, empathetic approach and are committed to offering a high standard of support.

Role of Community Outreach Support Worker

Clients will have their own individual support plans, which support workers will work towards; but support visits can include, but are not exhaustive to:

  1. Supporting clients to access services within the community, including appointments (This may involve transporting clients in their private car)
  2. Providing support to develop skills within the home, including cooking, cleaning etc
  3. Assisting with shopping requirements
  4. Providing support with budgeting / paying bills
  5. Providing company for activities such as walking
  6. Providing support with attending community events and appointments
  7. Recognise the individual needs of each client and help meet their social, emotional, and intellectual needs.

Please note this role does not involve any personal care or the administration of any medications.

The successful candidate will be required to hold a UK Driving licence and hold business insurance for their vehicle, as they are expected to support clients in the community.

This position is initially on a zero hour contract for a minimum of 6 months, but could lead to contracted hours, subject to service need.  Working days and hours to be agreed in response to client need.

 

Competitive Salary – NJC Scale 2 Pt 5/ Company Pension / Mileage Paid / Subsistence Allowance / as a bank worker you will receive paid holiday entitlement
