About our Outreach Service

Support can include but is not limited to:

Shopping, meal planning and preparation

Promoting physical and mental well-being

Accessing community resources and activities

Attending appointments

Social support

Budgeting

Advocacy

By visiting clients in the community, we are able to provide practical and social support to individuals living with Mental Health needs, including Dementia. Support can be in your own home or in your local community and can be a one-off visit, providing respite for a particular commitment or it can be more frequent such as weekly or fortnightly.

All of our staff are highly qualified and experienced, with backgrounds working in the NHS, Social Care and other Mental Health Sectors. They hold recent DBS Checks Certificates and receive regular training and supervision. If you think you could benefit from our Community Outreach Service, please contact us on 01749 670667.

Heads Up – Outreach Flyer 2020

"I live alone and I'm really isolated. I struggle with food shopping and managing my money. I don't get out of bed on the days that I don't have support from Heads Up. My outreach worker helps me to get up and motivated. I'm supported to plan meals and make a shopping list, and after lockdown she will help me learn to cook meals for myself. During lockdown Heads Up have delivered to me an Easter egg, cake & fruit. I've had a card made by a child and been given craft items. Heads Up Day Centre went into lockdown before I was able to start, but I'm really looking forward to it opening again. I'm really excited about doing pottery. It will get me out of my house, meeting people, and I'll feel less isolated."
